The landmark document “2021 ISHNE/HRS/EHRA/APHRS Expert Collaborative Statement on mHealth in Arrhythmia Management: Digital Medical Tools for Heart Rhythm Professionals” has been published in English (ref) and many other languages including Chinese with permission (ref). We believe that current and future efforts to implement mHealth in real-world practice as described here will contribute greatly to human health. Arrhythmia, especially malignant arrhythmia, serves as a major cause of disability and mortality worldwide, especially in the modern aging society. Suitable application of strategies to monitor and manage (and even prevent) these arrhythmic events by modalities introduced in this document will definitively improve general awareness and lead to ways to manage these events in daily life.
In our view, the key to success is economic viability, i.e., an mHealth system should have cost–benefit advantages. This will support availability, durability, and future improvements. Although investors may provide start-up capitals to establish the mHealth system, the next step to encourage at-risk populations (or payors) to join the system and pay a fee for the service may prove to be more challenging. To achieve that goal, the benefits of joining the mHealth system should be readily visible. Firstly, awareness in the broad population of such a system and its capabilities is necessary and the publication of this document in multiple languages is a good beginning. Then, the clinical benefits of mHealth need to be demonstrated. Next, participants should have a clear plan about what will follow after events are transmitted to the cloud. If the events need to be managed immediately, what are the SOPs? The message should be clear and the online instructions should be implemented by local medical professionals, who are near the individual signaling the event, and who can take the necessary actions to help the participants receive proper management. An added advantage is that, depending on the condition and the individual, mHealth can empower the participants themselves to manage their care. Knowledge of the benefits of joining the system by potential participants serves as the only solid reason for them to subscribe to an mHealth service. This fee may not be a financial burden for individuals if the population base is large enough (moreover a low fee may improve accessibility and erase the “digital divide”). However, a fee-based system (or reimbursement from payors) would sustain the system and promote its growth and improvement.
Here, we address the first step on mHealth, anticipating growing momentum in adoption and a bright future if we gain sufficient financial support from potential participants and various vendors of this system. The key path to success lies in our clear messagings and SOPs of this mHealth strategy aiming to enable participants living healthier and better.
